The agency agrees to:

1. Provide a safe work environment for offender and any tools and materials necessary for job assignment.
2. Provide supervision of work participants insuring that the offender is productive and working safely throughout the work assignment.
3. Schedule adult offenders to work on distinctly different days/times to avoid any possible scheduling overlap with students and/or juvenile offenders.
4. Treat offenders with dignity and respect.
5. Insure that proper medical treatment is administered when required and notify the community service staff immediately, when an injury has occurred.
6. Notify community service staff immediately, if offender fails to show, is continually tardy or is performing community service in an unsatisfactory manner.
7. Notify community service staff immediately and ask offender to leave, if offender consumes alcohol or a controlled substance while doing community service, or reports to perform community service under the influence of alcohol or a controlled substance, or has the odor of alcohol on his person.
8. Return the completed placement verification form to community service staff, when the completion date has expired or within five (5) days of completion of the required hours. Agency signature verifies that the information reported is true and accurate.
9. Accept no gratuities (gifts, money, agency donations, etc.) in exchange for crediting hours not actually worked or for falsifying the records. This is in direct violation of the North Carolina General Statutes and may be punishable by imprisonment, a fine or both. Notify community service staff immediately, if offender or agency personnel violate any of the conditions of this agreement or the conditions as set out in the Community Service Requirements nad Regulations.
10. Comply with all applicable local, state, and federal laws and adhere to fundamental values such as honesty, integrity, fairness, respect, trust, responsibility, and accountability.
11. The agency will follow ethical and accountable conduct in leadership, management, and operations as set forth by the N. C. Center for Nonprofits Standards for Excellence. http://www.ncnonprofits.org/standarsd.html

The Community Service Work Program agrees to:

1. Provide offenders, as available, to recipient agency with an explanation of offender's offense.
2. Inform recipient agency, if offender has any limitations/disabilities.
3. Notify recipient agency of changes relative to offender's reporting to that agency.
4. Promptly and effectively work with the recipient agency to resolve any problems that may arise as a result of work placement.
5. Provide medical insurance of accidental injury to the offender.
